Qualifications/Requirements for Applying:

Caregiver:

1. Must have 75 credit hours of training and a certificate of training as a
   home health aide, certified nurse assistant or direct care worker.
2. Must have at least 1 year on the job work experience in the health
   care or home care field.
3. Applicants applying for live-in position, must have 2 years experience
   and letter of recommendation preferred.
4. All applicants must have at least 2 work references.
5. All applicants must take a home health aide competency exam before
   completing an application.

LPN/RN:

1. State License
2. 1 year on the job experience
3. CPR/BCLS
4. Driver's License
5. Social Security Card
6. T.B. TEST
7. Physical Exam
8. Auto Insurance
9. Two employment references

Must complete medication exam/skills checklist and submit all required credentials upon completing application.
